International Launch: The interRAI Hospital Assessment Systems

Over the past 5 years, the interRAI research collaboration has developed and extensively tested a group of new assessment systems which support assessment, care planning, and quality improvement across the entire hospital continuum, from the emergency department (the interRAI ED), acute care (interRAI AC) to post-acute care and rehabilitation (interRAI PAC-Rehab).  Additional tools include brief screeners (the interRAI ED screener and interRAI Emergency Screener for Psychiatry (ESP)).  Two in-depth systems focus on comprehensive geriatric assessment (interRAI AC-CGA) and mental health (interRAI MH).  These systems share clinical observations, scales and screeners, enabling them to be used to support clinical care across the entire hospital continuum.

Of particular interest is the new interRAI Acute Care, designed to support the acute nursing admission process.  With less than 60 observations, it is vastly shorter than most existing hospital systems, which often exceed 300 items.  In spite of its brevity, it captures problems (delirium, dementia) and identifies risks (pressure ulcer, falls) with the same level of precision as traditional screeners.  This system promises to transform the nurse assessment process. It will feed valuable information to the entire multi-disciplinary team.
